How much does a session cost?

Our services range in price. Please contact 321 Performance for pricing.

Payment is required at or before the time of service. Our payment is based on 60 minute sessions with a Doctor of Physical Therapy. 321 Performance is considered out-of-network and can be considered for reimbursement (40-80% of cost) by your insurance company based on your out of network benefits. Please contact your insurance provider for your specific out of network benefits.

Do you offer payment options?

YES! We offer payment plans for up to 6 months after the initial visit.  We also offer a pay-as-you go option with agreement to scheduling your visits ahead of time.


Why is 321 Performance out-of-network for all insurance companies?

At 321 Performance, we believe that providing patients with one-on-one care is best for their rehab and return to their full potential. This type of treatment allows us to produce a truly individualized plan specific to your needs and provide the proper amount of attention to your issues. Typically, insurance dictates therapy treatment, with most in-network therapy clinics needing to see 3-4 patients an hour to be profitable. By being an out of network provider, we are able to not only address your specific movement limitations, but to work with you on your movement, mobility and strength so that you will be stronger and more durable when you leave us. Importantly, you’ll also better understand and become more confident in your own movements, and take those tools with you when you leave.

Do I need a referral to be seen at 321 Performance?

Florida is a “direct access state’. That means, a physician referral is not required for treatment by a physical therapist.  You are allowed to go directly to a physical therapist without a physician's referral first and receive treatment for 30 days.

After 30 days, we will need to get a treatment plan signed by an approved healthcare provider.  A referral can be given by an MD, Nurse Practitioner, Physician’s Assistant, or Chiropractor. Referrals are valid for one year.

Can I request reimbursement for physical therapy from my insurance company for out of pocket expenses?

Yes! Depending on your benefits, you can submit to your insurance company out of network therapy benefits. Forms are provided for you by your insurance company to be filled out and sent in along with needed receipts and treatment codes which can be provided for you by 321 Performance. 

Your reimbursement rate is set by your insurance company. Call your provider before you visit 321 Performance to confirm out-of-network therapy benefits as well as any pre-authorization requirements.

What is Dry Needling?

Dry needling is an advanced technique using thin, monofilament needles. Needles are inserted into muscle tissue to help decrease myofascial pain, address trigger points, increase muscle activation and help with recovery post sport.

Do I have to Crossfit to be seen at 321 Performance?

Absolutely Not! While Brian loves working with CrossFit athletes and helping them optimize their functional movement patterns. he treats any and all musculoskeletal injuries. Brian specializes in performance physical therapy and rehab.  He treats any and all musculoskeletal injuries including but not limited to:
